The Berkshire Healthcare Foundation Trust is urgently recruiting for a band 6 Senior Mental Health Practitioner in the Berkshire Early Intervention in Psychosis Service; this is a multi-disciplinary team consisting of care co-ordinators, psychologists, psychiatrists, pharmacist, STaR and IPS worker and team lead operating in a locality where there is a diverse community with a high level of integration between mental health services.

Location:The Old Forge, Wokingham


Shifts/Hours:30 hours per week; The service operates Monday to Friday 09:00 to 17:00. Shift pattern is to be confirmed

Band 6 Rate:

  • Day Rate - £19.38 per hour
  • Night/Sat Rate - £25.19 per hour
  • Sun/Bank Holiday Rate - £31.00 per hour

Job Role

Conducting assessments, medication management, providing high quality treatments/interventions in line with a NICE concordant EIP service, care co-ordination and panel applications are some of the core activities of the post. Working collaboratively with other disciplines and other teams in the locality are essential to ensure a comprehensive service is provided to the local population. As one of the senior members of the team the post holder will need to act as a role model and be able to manage high levels of responsibility.

Must haveresponsibilities include:

  • To have clinical responsibility for a case load as specified by the EIP Clinical Lead.

To assess service user needs, plan effectively, monitor and evaluate interventions using evidence-based practice which is relevant to individual needs and within a framework of research and audit.

To work with carers and family members, offering carers assessments and providing support and information to them as indicated. Signposting to third sector agencies to ensure they receive appropriate advice and support. Advising them about the triggers around risk factors relating to the service user, within the boundaries of confidentiality.

To monitor; risk, progress and where necessary compliance including the service users views to their care and treatment and when appropriate to work in partnership with the service users family and carers. To explain and discuss medication with the service user, including side effects they may experience and the role that the medication may have in their recovery or ongoing care.

To work in partnership with the individual service user and their families and carers as appropriate to ensure the delivery of the care plan, and to enable the development of a plan to facilitate their safety, promote their well-being and support their independence and inclusion in the community, by using recognised age-appropriate self-management tools where applicable. This may include working closely with partner agencies such as voluntary sector agencies, local authority, charities, and colleges which may be able to more appropriately help the service user to meet their needs.
