Due to an expansion of our team, an exciting opportunity has arisen to join our Long-Term Conditions Services, working with our established Falls Prevention Team within one of our East Surrey primary care networks. The network covers 3 GP practices, and the role will entail the provision of clinical care for patients referred following a history of falls. Patient assessment and care will be delivered in patients’ homes and the postholder will work closely with the practices with the PCN’s multi-disciplinary team (MDT) to meet the needs of patients and carers.

Service evaluation and development will be integral to this role and support will be provided by the Band 7 OT Team Lead. This is the second PCN post to be developed, providing an exciting opportunity for OT expansion into GP practices.

We are looking for a part time Band 6 Community Occupational Therapist to join our existing team. You will need to be able to work independently, making autonomous clinical decisions and appropriate referrals to other services.

Central to this role are:
- strong communication skills
- expertise and experience in provision of care for patients with a history of falls, reduced mobility and/or musculoskeletal impairments
- experience of supporting service development and evaluation

You must be a licensed car driver and have access to a car for this post.
