Are you an Advanced clinical practitioner looking for an opportunity to work as a community matron in the community setting . Community matrons work in primary care networks supporting patients in their own homes who have complex health needs . The community matron works closely with the wider community nursing team and GP practices within the PCN .

As part of our service we support and train Advanced clinical practitioners supervised and assessed by community matron colleagues , we are looking for a community matron to work peripatetically across three pcns supporting those community matrons who are working with Advanced clinical practice students , depending on your hours you would spend a minimum of one day in each of the three PCN's team undertaking community matron visits to patients in their own homes .

This is a great opportunity to gain experience across different pcn's meeting a variety of patients with different complex needs .
