Care Home Support Nurse, East Berkshire

We are looking to recruit a band 7 autonomous general nurse to support and empower care homes staff across East Berkshire through education, clinical advice, and signposting. This is a flexible Monday to Friday role working from 8:30-4:30pm from various locations including working from home. We are looking for someone who is passionate about developing staff and improving outcomes in the ageing well agenda. The roles require someone with great interpersonal and leadership skills and able to demonstrate forward thinking ideas to enhance care for care home population. As an autonomous practitioner, you should be able to demonstrate effective management skills and be responsible for innovative practice as well as be open to take advantage of the developmental opportunities available. Working in collaboration with care homes, local authorities, and CCG, we aim to proactively enhance residents and empower staff in East Berkshire care homes.

About the Job

  • To work in partnership with care homes to develop pro-active training/educating advisory service that aims to improve the health, well-being, and independence of all care home residents
  • Identify the specific skills and training requirements for each care home and deliver and record a tailored training programme to include a train the trainer option
  • Attend and contribute to clinical/care homes MDT and cluster meetings where appropriate.
  • Share expert knowledge and skills with the care homes to enhance their skills.
  • Continue working alongside local authorities to support those homes rated red or amber and those with residents who have frequent hospital admissions.
  • Signpost and support with making direct referrals on to other community services or professions as required

The 3 “must haves” for you to be considered for this role:

  1. Senior registered general nurse with supervisory/management experience
  2. Experience in facilitate education and training for others
  3. Car driver with ability and readiness to travel to multiple sites
