The Health Case Management Service provides a case management function for the population of Leeds (18 years and above) who are eligible for NHS Fast Track and Continuing Healthcare funding; this includes hospital and within their own home, over 7 days, Monday to Friday 08:00 to 20:00 and Saturday and Sunday 08:00 to 18:00.

The Health Case Management Service is a citywide service. The teams consists of Health Case Managers, Health Case Management Coordinators and Health Case Management Assistants who are professionals with health and social care backgrounds and have a wealth of experience in all aspects of health case management.

The Health Case Management Service (HCMS) consists of professionals from nursing, social work or Allied Health Professional backgrounds, and provides a specialist case management function for adults with high level health and social care needs. It would be essential for applicants to have Best Interest Assessor qualification or have the aspiration to undertake the training.

We are looking to recruit a Health Case Manager who will work closely with staff to ensure timely, holistic and person centred assessment, care planning and care co-ordination is provided to all service users. Close working with the Neighbourhood Teams, Hospital Social Work teams and Continuing Health Care Service within the CCG will also be essential.

The post holder will join a friendly and hardworking team which is committed to providing a high quality service. You will have experience of working with adults and their carers within a hospital and /or community setting. You will have excellent organisational skills and be able to work on your own independently and creatively using a problem solving approach, as well as part of a busy team. You will have experience of effective case management of adults with complex needs, and those at the end of life.

The post holder will be line managed by a Clinical Quality Lead and will receive access to regular supervision and personal development opportunities.

If you are successful in being offered a position which involves working with patients, you will be required to undertake a Disclosure and Barring Service (DBS) check. The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost from your salary. There will be no charge if you are enrolled in the DBS Update Service.
