This post is part of a joint initiative between CLCH and Imperial College Healthcare NHS Trust (ICHT).

We are looking to appoint a Head of Integrated Discharge Hub to work across the ICHT sites. The role will work with all the Organisational Partners associated with patient discharge Hammersmith & Fulham, Westminster and Kensington & Chelsea to ensure that patient flow is rapid and effective. The post holder will also be required to manage the Discharge Hub team.

The Head of Integrated Discharge Hub will work very closely with acute Trusts to ensure that the delivery of system flow meets the needs of the ICS. This role will report to the Assistant Director of Discharge System Flow for CLCH and the Integrated Care Programme Director for Imperial College Healthcare NHS Trust.

The successful candidate’s duties include:

  • Provision of senior leadership to the Integrated Care team
  • Use matrix management skills to lead teams from a range of local partner organisations
  • Implementation of a revised Integrated model to increase flow. Building on the existing work that has been undertaken on Discharge to Assess (D2A) deliver a fully integrated discharge team working across health and social care
  • Daily management and challenge of the identification of medically optimised, stranded and super stranded patients.
  • NW London and trust level problem-solving and unblocking issues with partner organisations to ensure delays are resolved as swiftly as possible, escalating to executives where necessary
  • Escalation and reporting of issues relating to clinical risk inc. ensuring lessons learnt and necessary improvements made.
  • Leading capacity planning and assurance processes for key events, including bank holidays, summer holidays and winter, reporting to regulators where required
  • Promoting shared learning and reflection to enable team members to resolve issues internally
  • Attending and providing system leadership to MADE events held at the relevant acute hospital
  • Providing accurate reporting data to executives (and regulators where required)
  • Accountable reporting to key governance forums including the A&E Delivery Board and Discharge Steering Group
  • Monitoring, reporting, and acting on key trends and themes identified through the discharge data sets
  • Development, sign off and implementation of a system-wide discharge policy
  • Development, sign off and implementation of standard operating procedures incorporating agreed pathways out of hospital (including CHC, any community bedded capacity and the neighborhood community teams)
  • Development of a functional discharge reporting system that supports effective operational delivery and highlights areas requiring escalation and unblocking
  • Providing support to the integrated discharge executive including developing agendas, reports and actions and providing timely data and analysis

The successful candidate will be expected to act with a high degree of autonomy, will ensure that key deliverables are prioritised and completed in agreed timeframes. Demonstrating well developed political and interpersonal skills, the post holder will be able to hold work stream leads to account in a positive and constructive manner.
