District Nurse Team Lead

Fixed Term position (Until 31st March 2022)

Are you an experienced community nurse or qualified district nurse, looking for the next step in your career to take on the role of District Nursing Team Lead?

We are looking to recruit a Band 7 District Nursing Team Leader to join us in our Central London community nursing service in West London which covers Kensington & Chelsea and North West Westminster.

The post holder will have responsibility for running a team of district nurses, community nurses, health care assistants, and Nursing associates to deliver care to people in their own homes. The role is varied, with opportunities to develop both clinical and management/leadership skills.

The post holder will manage the care of patients with long term conditions, incorporating monitoring progress and promoting the preventative aspects of nursing practice.

The post holder will be expected to undertake the holistic assessment of patients using a person centred approach, and co-ordinate care packages across both health and social care as appropriate to avoid inappropriate admission to hospital or long term care environments, in addition to facilitate the timely discharge of patients from secondary care whilst taking active part in innovative projects that enhance patient centred service delivery and development. The DN is responsible for promoting good teamwork and providing teaching and support to other team members.
