You will be employed by Southern Health Foundation Trust and seconded out to Hampshire County Council, initially on a 6 month fixed term contract.

You will be supported daily by our valuable nurses and therapists, with further opportunities for personal development within the Trust.

Hampshire County Councils' Adults Health and Care provide a range of residential care and nursing services for older people across the county, some of whom are living with dementia. Clarence Unit supports adults with complex health needs following their discharge from hospital.

No two days are the same in this varied and rewarding role. As a general guide, your duties may include:

• Supporting individuals with personal care
• Support with hydration and nutrition
• Moving and positioning
• Accurate record keeping
• Undertaking activities such as entertainment and recreation to enhance the wellbeing of individuals
• Supporting Individuals with medication

As an employee, you are accountable to ensure that you know to how to respond when you are concerned for the safety of a child, young person or adult at risk, applying the principles of the Family Approach to ensure evidence based analysis, risk management and appropriate action and escalation. SHFT require staff to work effectively together using the Family Approach to achieve better outcomes for adults, children and families. The Trust will support you in this duty by providing ongoing training, support and advice and have a Corporate Safeguarding Team who can be contacted for guidance and the provision of safeguarding supervision.
