Bolton NHS Foundation Trust was formed in 2008 and, in 2011, we became an integrated provider with community health services joining the trust. Covering a catchment area that also incorporates parts of Bury, Wigan and Salford, we deliver hospital and community based services from the Royal Bolton Hospital in Farnworth and from 20 health centres across Bolton, to a catchment population of 320,000 people. We have a CQC rating as good overall and we have some key areas of outstanding practice with an outstanding rating for being well led at every level.

Bolton NHS Foundation Trust is an integrated provider of acute, intermediate and community health care serving the population of Bolton. The Family Care Division aims to deliver the highest standards of care in maternity, sexual health, gynaecology and a range of acute and community children’s services. We deliver over 6,000 babies and carry around 1,500 gynaecological procedures each year. The Trust also has a tertiary level Neonatal Intensive Care Unit, enabling 1 in 4 women across Greater Manchester to receive in-utero transfer and birth at extreme premature gestation at Bolton

An exciting opportunity has arisen for experienced paediatric registered nurse to be part of the Integrated Community Paediatric Service (ICPS) to support Children and Young People (CYP) in the community with complex, continuing nursing needs, some of who also have Continuing Health Care funded packages of care . The service also supports children and families with the delivery of end of life care in the community if this is their preferred place of care.

We are looking for a motivated and enthusiastic nurse who will support ICPS Matron and Team Leaders to deliver high standards of evidenced based care for Children and Young People (CYP) within the ICPS. Working within the Continuing Care team, a significant amount of time will be as an autonomous practitioner in a variety of community settings, with support from colleagues and team leader.
