This is an exciting opportunity to join NHS Trafford CCG as Head of Personalised Care.

 We are looking for a confident strategic leader to lead on the development and redesign of integrated systems and processes relating to the delivery of Funded Nursing Care (FNC) and Continuing Care Functions (including, but not limited to, NHS Continuing Healthcare and Personal Health budgets (PHBs) services), and the clinical commissioning and case management elements of mental health and learning disability placements (IMHaD).

You will need the ability to programme direct the required innovation and change in service in a planned and structured manner in collaboration with senior Local Authority colleagues and other partners. An ability to adapt to a dynamic, fast moving and developing organisational environment will be necessary as the CCG moves into system reform and into Integrated Care Systems increasingly integrated structures.

You will lead at a senior level working across both health and care agendas to build a strong and robust system of personalised care, leading NHS Continuing Healthcare, FNC, PHB and  IMHaD, teams within the CCG. This is a complex area of work and will need a leader who is able to respond to a very dynamic service, identifying and leading innovation for the benefit of the service and population.

This post holder will report to the Deputy Chief Nurse and present factual and accurate reports both into the CCG structures and also into NHE England.

We are looking for a dynamic individual with a strong proven track record of leadership, teams’ management and robust experience in all areas of personalisation putting the best interests of our residents at the centre of all we do.

In Trafford we have a robust locality plan which is supported by our Strategic Design Groups, the post holder will be expected to support our shared vision where we aim for all our residents to live well at home, wherever that home may be.

About Us

During 2019/20 the CCG has concentrated on establishing a clear strategy and direction for the health and social care system. The 'Trafford Together Health and Social Care Locality Plan for 2019-24' has been agreed by all our partners in our Local Care Alliance (LCA).

The LCA was established in 2018/19 and is an alliance of providers and commissioners in Trafford who are working together to enable the triple aim of improved outcomes, improved services and improved resource utilisation. The LCA plays an important role in our system and continues to be led by our independent Chair. As we move towards system reform this Board will work towards the new Integrated Care System models, it is an exciting time to work within this area of healthcare.

 

The current LCA includes the CCG, Trafford Council, Manchester University Foundation Trust (MFT), Greater Manchester Mental Health NHS Foundation Trust, Thrive Trafford, Pennine Care Foundation Trust and Primary Care Providers. Partners have signed a Memorandum of Understanding agreeing to work together to deliver services and to help achieve improved health and social care outcomes.
