Southern Health NHS Foundation Trust plays a key role in supporting the discharge and ongoing care of patients from our local acute hospital. In order to improve the patient experience we have been asked to provide support to a programme of working relating to Medically Fit for Discharge patients in the South East area of Hampshire areas.

This means we have a number of exciting patient focused roles available to meet an increase in patient numbers so if you have the skills and aptitude to really do something wonderful today them look no further.

Many HCSW’s have gone on to Band 4 Associate Practitioner roles or undertaken Nurse training to become qualified. In this role you will be supported in your Care certificate and to attend training courses as part of your ongoing professional development.

The multidisciplinary Enhanced Recovery and Support at Home team based at Aerodrome House are looking to recruit HCSW’s to become integral part of our rehabilitation and re-enablement service covering a wide area. You will be part of a busy team providing varied day to day, patient focused short term packages of care working eight hour shifts over a seven days between the hours of 8am and 9 pm. You will support our extensive team of OT’s, physiotherapists and nurses in providing community based support usually, although not exclusively, to the over 65 age group within their own homes. Access to a motor vehicle and a clean driving license are essential for this role. Mileage will be paid monthly and uniforms are provided.

You will comply with all Employer processes in order to provide yearly permission to the Employer enabling it to view your status and to undertake random checks as required. If you change to a role where a check would no longer be required this will be confirmed and you may end your membership if desired.
