Home First - Crisis Response Team

Work Pattern:

37.5 hours per week, 7 day working rota.

This is a band 6 trainee ACP role. On successful completion of the Non-Medical Prescribing (NMP) and Advanced Physical Assessment and Consultation Skills (APACS) modules the post holder will progress into a band 7 post.

The Post:

This is a new and exciting opportunity for a Physiotherapist to join the existing Crisis/Rapid Response team in Doncaster. The current service has four Advanced Nurse Practitioners who lead the triage process and provide the initial assessments of patients in a health or social care crisis at home. Developing an ACP Physiotherapist with the necessary skills to also provide this intervention will expand the remit and skills within the service from a therapy perspective.

The service operates 7 days a week and there is an expectation that this practitioner will contribute fully to this rota with weekend working every 4-5 weeks.

As well as clinical expertise, this practitioner will be responsible for clinical leadership and development of other staff who work within the Crisis Response service and the wider Home First pathway. The successful applicant will need to have excellent communication skills as networking with the many partners within Doncaster is an essential part of the role. This would include GP’s, Social Care, YAS, Voluntary Sector.

This individual must demonstrate the RDASH values and beliefs. Placing the patient at the centre of their decision making is imperative as is the provision of high-quality care. The individual must be confident in their skills and abilities and be prepared to go into highly complex patient situations to problem solve and ensure patients are safe to be left at home. A forward thinking and innovative individual will have the ability to work within the team and strengthen the crisis response offered by Doncaster.

This is a trainee post so the successful applicant will be supported to achieve the clinical skills, competence and confidence to fulfil the expected role.
