CLCH are looking for GPs interested in undertaking an exciting new role in our Complex Case and Frailty MDT service in West Herts

 

The CLCH Complex Case Management pathway, including Frailty Clinics Locality Frailty and Complex Case MDT forum commenced in West Herts in August 2020.

 

The aim of both is to plan and coordinate joined-up and integrated care to adults who are frail, have complex needs or are high intensity users of services.

 

We are looking for a dynamic, pragmatic GP to be a regular member of our Locality MDT forum, providing community Medical expertise and oversight to the review and management planning process. The MDT forum brings together professionals from a range of providers and specialisms to provide a comprehensive case discussion, to develop coordinated care plans and provide advice and guidance on the management of the most complex cases in the locality. This proactive care model supports the avoidance of preventable hospital admission in the long term.

 

MDT SET-UP

 

MDTs run in each of the 4 West Herts Localities on a weekly basis.

Each MDT is 4 hours in duration, with a planned review of up to 12 patients per meeting.

MDTs will run 51 weeks a year.

 

The sessions are currently being delivered remotely using Microsoft Teams following the timetable below. There is the option in future to revert to face to face meetings at the venues also outlined below.

 

The weekly timetable for MDT meetings is:

Dacorum Locality – Tuesday mornings (Hemel One)

St Albans and Harpenden Locality – Wednesday mornings (Harpenden Memorial Hospital)

Watford and Three Rivers Locality – Thursday mornings (BRE, Garston)

Hertsmere Locality – Wednesday afternoon (Potters Bar Community Hospital)

 

We are looking to recruit a number of GPs on a 12 month fixed term contract to become an integral part of these locality MDTs. In order to maintain the service throughout the year, it is suggested that at least two individuals per locality are required working on an alternating basis and providing prospective holiday cover.
