Are you looking to develop specialist skills? Would you like to work in a developing and exciting Community Nursing Service that supports Primary care and facilitates faster discharges from the acute sector by supporting patients in their own homes?

We are looking to recruit a Community Staff Nurse with an interest in diabetes who will contribute to the delivery of quality driven and safe care to patients with diabetes in Portsmouth. We are a contemporary and forward-thinking Community service based in Central Portsmouth. Our working base is at St Marys Community Health Campus in Milton, Portsmouth.

Our ideal candidate will be a Registered Nurse with an interest in nursing patients with diabetes. You will work alongside the Community nursing team in delivering daily diabetes care which will include review and assessment of patient needs and support.

You will be expected to support the localities nursing teams and work alongside the senior Diabetes Link Nurse to support educational and preventative programmes.

You will need to be flexible, forward thinking and a team player.

You must hold a full valid driving licence and have access to a car.
