South West Yorkshire Partnership NHS Foundation Trust in partnership with Calderdale Council has three multi disciplinary Community Mental Health Pathways within Calderdale.

We are looking for a committed, experienced and suitably qualified mental health practitioner to work in the Lower Valley Enhanced Pathway managing a caseload of service users with severe and complex mental health difficulties in community.

Mental Health Services within Calderdale are jointly managed and all service user records are maintained on a shared IT system. The Pathways comprise of Nursing, Social Work, Support, Psychology and Medical Staff as team members and operate to new ways of working. Caseloads are comprised of working age adults experiencing complex mental health needs. Staff supervision, support and profession development are a high priority.

Experience of working within the community would be advantageous.
