Are you an experienced Senior Mental Health Nurse seeking an exciting and unique new role? Due to the high level success of a recent pilot there is the opportunity for significant growth in this diverse, patient facing role.

 

Working closely with primary care networks in a designated GP surgery you will support the integrated, joined up working for the population we serve. Breaching the gap between primary care and secondary mental health services will in turn improve the service user experience across Southampton.

 

The successful candidate will be enthusiastic, resourceful and committed to providing client centred, recovery focussed care. You will be assessing, providing support and therapeutic interventions to service users within a GP Surgery. Within your role you will also be liaising with and signposting to other services and working closely with the CMHT regarding referrals, advice, information sharing and for clinical supervision.
