Solent NHS Trust is working in partnership with Portsmouth Hospital University Trust to build a flexible, adaptable, confident and competent workforce, who can support seamless pathways and improved patient journeys for children between acute or specialist hospital care and community care. As part of this exciting long rotation, we are looking for a well-motivated, enthusiastic individual to join our Integrated Community Childrens Nursing service in Portsmouth.

Our established, passionate and dedicated integrated Community Children’s Nursing Service (iCCNS) works collaboratively with children, young people (CYP), families and the wider multi-disciplinary team to ensure safe care is provided within the community setting wherever possible. We deliver high quality, holistic and child centred care to CYP who require nursing intervention, across Portsmouth and the South East Hampshire area with acute and long term conditions, complex needs, life-limiting and life-threatening illness, including CYP requiring palliative and end of life care.

This post will enable you to consolidate your Children’s Nursing training whilst further developing skills and knowledge through experiencing various aspects of nursing care to children and young people across a range of acute and community settings. The post will offer rotation across Solent NHS Trust Integrated Childrens Community Nursing Service and the Children’s departments in Portsmouth Hospitals University Trust.

Your development will be supported throughout your preceptorship year by a named preceptor, phased inductions and the honouring of protected training opportunities and attendance at the PIER and Solent NHS Preceptorship programmes. You need to be motivated, forward thinking and have a sound professional knowledge and excellent communication skills. You should have the ability to prioritise and work both independently and as part of a team. Following your rotation, your employment will continue in the ICCN service as a Band 5 CCN.

This post requires youhold a full UK driving licence and have access to the use of a car.
