The band 5 opportunity is welcoming candidates who exhibit the values and behaviours of Leeds Community Healthcare and can demonstrate attributes required for the role; day to day coordination responsibilities which include assessment, delivery and evaluation of person centred care within your domain of competence and knowledge.

You will manage a caseload. We encourage applicants who hold an NVQ Level 4, professional degree or equivalent, have a knowledge of health and social care in particular around the service area and profession. Additionally you should demonstrate a wide variety of communication skills and have experience of working as part of a wide multi-disciplinary team.

If you are successful in being offered a position which involves working with patients, you will be required to undertake a Disclosure and Barring Service (DBS) check. The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost from your salary. There will be no charge if you are enrolled in the DBS Update Service.
