Paramedic Practitioner (Urgent Care Practitioner) – for a primary care visiting service to support with home visits

The role:

  • all our surgeries offer urgent care sessions for patients who feel they can’t wait for a routine appointment. You will work closely with GP's and other allied health professionals
  • you will be responsible for providing face to face consultations / home visits for patients presenting, unscheduled, with a wide range of minor injuries, long term conditions and minor illness needs.
  • you will act as an autonomous clinician and take responsibility for the assessment, treatment and/or to appropriately refer patients to specialist services when required
  • be compliant with the NMC or HCPC Code of Professional Conduct and any associated national guidelines and abide by their guidance.
  • take a comprehensive patient history, carry out physical examinations and use expert knowledge and clinical judgement to identify the potential diagnosis
  • refer patients for investigations and/or the appropriate specialist where appropriate
  • make a final diagnosis
  • Independently decide on and carry out treatment, including the prescribing of medicines
  • use their extensive practice experience to plan and provide skilled and competent care to meet patients’ health and social care needs, involving other members of the wider health care team as appropriate;
  • ensure the provision of continuity of care including follow-up visits;
  • assess and evaluate, with patients, the effectiveness of the treatment and care provided and make changes as needed;
  • work independently, although often as part of a local and wider health care team;
  • provide leadership to the wider team and make sure that each patient’s treatment and care is based on best practice

What we look for in your application 

  • Registered Nurse with the NMC (Nursing and Midwifery Council) or a qualified paramedic registered with the HCPC (Health and Care Professionals Council)
  • 2+ years proven record of acting as an paramedic practitioner / urgent care practitioner dealing with patients presenting with a wide range of medical issues
  • Background of working in a fast paced Primary Health Care or acute environments is essential 
  • You will have a proven record of acting in roles where you have acted autonomously  and responsibly for the assessment, treatment and/or appropriately refer patients to specialist services when required
  • A recognised qualification in Minor injury and Minor Illness is essential.
  • Proven background of dealing with a diverse range of patients from all walks of life
  • Proven background of building positive rapport with a diverse range of internal colleagues and clients
  • Proven ability to work autonomously whilst also having an awareness when identifying the need to access the wider clinical team or external agencies appropriately and safely
  • First class organisational and time management skills in a fast paced environment is essential.
  • Hold a Full UK Driving License and access to a vehicle to travel for home visits, between surgeries and other health care settings as appropriate
