HL are an established and expanding healthcare organisation responsible for providing a variety of urgent and primary health care services across Hampshire.

As anAdvanced Nurse Practitioner, (ANP or APP) to join the Extended Access and Urgent Primary Care Service at PHL to join the team based in Lymington.You will join us on a Bank, evening and weekend basis.

Hours:Monday & Tuesday evening 1830-2130, Saturday & Sunday 1300-1700

Advanced Nurse Practitioner Duties and Responsibilities:

  • Your duties will include aspects of the patient journey including initial history taking; clinical physical assessment; diagnosis; treatment; prescribing medication and evaluation of their care.
  • Through telephone clinical assessment, obtain a relevant and sequential history of the presenting complaint to determine a working diagnosis.
  • Formulate a management plan for the patient, using other sources of healthcare professionals (such as GP or Pharmacist) if required to support diagnosis or next course of action.
  • Prescribe medication as required within your clinical competence.
  • Recognise the need and initiate emergency treatment to save life or to safeguard the interests of the patient and refer to emergency services when appropriate ·
  • To be professionally and legally responsible and accountable for all aspects of clinical decision making within your clinical competence

Advanced Practitioner Skills & Experience:

  • 3 years of experience working in a primary care GP practice.
  • 3 years’ experience in an urgent care setting such as an urgent treatment centre.
  • At least 6 years post qualification.
  • Excellent knowledge and experience in the assessment and management of minor injury and illness presentations.
  • Good knowledge of primary care referral pathways such as 2-week-wait referrals
  • Registered with the NMC/HCPC
  • Non-medical prescriber
  • Post graduate diploma (PgDip) and working towards completion of the MSc qualification. Completed level 7 modules must include a history taking and patient examination/decision-making module.
