Southern Health is one of the largest Foundation Trusts in the UK, specialising in mental health, adult and child community health and learning disabilities.


The Older Persons Community Mental Health team, based at Havant Health Centre, and which covers, Havant, Hayling Island and Waterlooville is looking for a Nursing Associate who will work as a core member of the Community Mental Health team to co-ordinate the care of a complex service users. The appropriate candidate will be joining the team in a fantastic new role as we start a physical health clinic for functional and organic mental health patients, as well as completing follow up and initial assessments.

Working under the supervision of the Team Manager or their nominated deputy, you will formulate care and risk management plans in accordance with the identified need of the patient and ensure that these are recovery focused and developed in collaboration with the service user and where appropriate their relative/and carer.

You will have a role to play as we aspire to provide excellent patient care. We are looking for people who are able to demonstrate a passion for the care of patients and their families. If you have an interest in caring for patients with functional mental health needs or dementia then this is the role for you!

You will be a registered Nurse Associate, with post registration experience.

As the post is working in the community a valid driving licence and access to a vehicle for work purposes is essential.
