We are looking for several experienced nurses with diabetes experience or someone already working as a Diabetes Specialist Nurse to join our growing service. We have been lucky to have been granted additional funding to develop our service to meet the growing needs of people living with diabetes within Southampton City. This is an exciting opportunity to join a dynamic and motivated service where successful candidates will be provided with mentorship and personal development opportunities.

The work base will be at The Royal South Hants Hospital in Southampton and our service hours are Monday to Friday 9am -5pm. We would consider applications from full or part time applicants.

The Solent NHS Trust’s Community Diabetes Specialist Service is committed to making a difference to the lives of people living with diabetes by keeping them safe and well. Also, keeping them well at home and avoiding the need for hospital admissions. We work closely with a variety of service providers such as Primary Care, Community Nursing teams, Urgent Response Service, SCAS, mental health teams- such as, Steps to Wellbeing and more.

For this post, you will need to hold a full UK driving licence and have access to the use of a car

Job Responsibilities

Provide diabetes specialist nurse support, advice, education and information relating specifically to diabetes and diabetes care, to health professionals, patients and carers across the primary and community care setting.

To meet the needs of people living with diabetes and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. Care is based on comprehensive assessment, ensures continuity and is patient/client centred, culturally sensitive and evidence-based and which promotes independence.

Responsible for influencing clinical decisions and monitoring the quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to own field of practice, underpinned by theory and expertise.

Manage a caseload/named nurse

Supervise junior members of staff, students on placement.

Personal skills/qualities

The successful candidate would have excellent interpersonal skills and show enthusiasm and passion for caring for people living with diabetes. They will also have a positive attitude towards to problem solving. Being able to work in and interdisciplinary team, work collaboratively with different organisations would also be required.
