WORCESTERSHIRE Acute Hospitals NHS trust (WAHT) is a large organisation providing acute services to a population of 580,000 people from the Alexandra Hospital in Redditch, Kidderminster Hospital and Treatment Centre and Worcestershire Royal Hospital, as well as caring for patients from surrounding counties and further afield. We have an exciting opportunity for qualified or trainee Advanced Clinical Practitioners with non-medical prescribing and advanced adult health assessment qualifications to join our GEMS (Geriatric Emergency Medicine Service). Our GEMS has an enthusiastic and motivated multi-disciplinary team of Advanced Clinical Practitioners, Pharmacists, Nurses, Physiotherapists, Occupational Therapists, and Social Workers who work alongside Consultant Geriatricians to deliver Comprehensive Geriatric Assessment at the busy front door of the acute hospital and in-reaching to our inpatient areas to embed a frailty sensitive ethos.

 

Working as part of Worcestershire’s ICOPE (Integrated Care of Older People) pathway, in an emergent integrated care system we are developing proactive models of care to meet the health needs of older people with frailty and complex health needs. We use the Rockwood Clinical Frailty Scale to identify frailty and with Comprehensive Geriatric Assessment (CGA) the team aim to facilitate same day discharge and avoid unnecessary hospital admission.
