About the Role   A fantastic opportunity has arisen for an Advanced Nurse Practitioner to join our friendly team on a part-time/full-time basis. We are a 6 partner practice with approximately 14,500 patients. Our team includes 5 practice nurses and 1 HCA. We operate a system-one clinical system. 
The successful applicant will be an autonomous practitioner able to work independently and in conjunction with other health care professionals to:
  • Prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner and with GP support.
  • Deliver a high standard of patient care, assessing, diagnosing and treating patients’ conditions, face to face and via telephone consultations’.
  • Assess, diagnose and treat patients with a range of acute and non-acute medical conditions.
  • Prescribe medication as defined by current legislative framework and practice policy.
  • Visit patients either in their own home or in a care home environment at times when it is necessary.
  About the Candidate

Must be a registered nurse who holds a prescribing qualification and have experience of diagnosing health conditions.

Have good communication and IT skills and be a flexible team player.

Experience of working within a Primary Care setting is desirable, but not essential.
