Seaforth & Litherland Primary Care Network is a dynamic Network of nine General Practices, located within South Sefton. The PCN formed in 2019 and has developed local partnerships and governance structures rapidly, against a challenging primary care backdrop which has been characterised by the COVID pandemic.

The PCN has built a successful multi-disciplinary team in partnership with the GP Federation, the South Sefton CCG, and local voluntary services, providing measurable benefits for the population of just over 40,000. This includes Clinical Pharmacists, Social Prescribing Link Workers, and a First Contact Physiotherapist.

As with General Practices, Primary Care Networks have certain requirements they are mandated to fulfil, which fall under the Network Contract Directed Enhanced Services. To contribute towards the delivery of these requirements Seaforth & Litherland PCN is recruiting four Care Co-ordinators, to be based within General Practice.

This position will be patient-facing, and the successful candidate will take a leading role in helping to shape the PCN’s personalised care approach, as part of a Network-wide team. The post-holders will proactively identify and work with patients to provide co-ordination and navigation of care and support across health and care services. They will work within their host practice to review the needs of the patients on their caseload, with the ultimate aim of improving their quality of life.

There will be significant multi-disciplinary working, with the post-holders interacting with the PCN’s team of Additional Roles to provide a holistic approach to personalised care.
