An exciting opportunity is available at The Rotherham NHS Foundation Trust for the post of Discharge Liaison Officer for Ward A4.


The role of the Discharge Liaison Officer role is to work alongside the ward teams in supporting the multi-disciplinary team in the safe, effective, well led transfer of care from hospital to the patient’s destination on discharge. This role will provide support to the Registered Nurse who holds accountability and responsibility for the discharge of patients.


We are looking for colleagues with excellent communication skills, can work effectively as part of a team, with our patients and their families/carers.


The role will be ward based with effective working relationships with the trusts Integrated Discharge Team.


The role at present is predominately Monday to Friday and ward based however there is an expectation that the Discharge Liaison Officers work as a team and cross cover on other wards, (there is a possibility in future that these roles will include some weekend working).

The cost of undertaking a DBS disclosure will be met by the individual. The Trust will meet the initial costs of the disclosure, and you can choose to have this deducted from either your first month’s salary or in instalments over your first 3 months’ salary. The current price of a standard check is £23 and an enhanced check is £40.

New starters to the NHS will start at the bottom of the advertised salary band.
