MLCSU has been commissioned by Derby and Derbyshire CCG to develop and deliver an Enhanced Case Management Service for 4 Specified Patient Cohorts (Mental Health, Learning Disability, Autistic Spectrum Disorder & Children & Young People) with a proposed mobilisation date of 1st April 2021.

The Enhanced Case Management Service will be housed within our friendly and supportive Continuing Health Care and Complex Care Needs Team.

The service is a twin site operation [Chesterfield and Derby] but is currently a remote working operation in line with COVID19 Restrictions.

This is an exciting time to join a dynamic and growing patient centred service focused on delivering exceptional care services to Adults and Children and Young People presenting with Complex Care Needs.

Can you help us make the difference?

The 4 Patient Cohorts are:

  • Transforming Care
  • S117 Aftercare duties under the Mental Health Act 1983
  • CYP [ Section 75 shared duties with Local Authorities] And
  • Dowry Retraction / Legacy cases

Who we’re looking for:

  • Scheduling Administrator to arrange/schedule patient care assessments and reviews for complex case management clinical colleagues, including contacting individuals, family members/representatives and other relevant professionals, as required.
