Triage Nurse – community Intervention service

An opportunity has arisen for an experienced Band 6 triage nurse to join the Cannock Community intervention Team . We are looking for a dynamic, highly skilled, Nurse to join our team based In Norton Canes Medical Centre, working alongside the Locality Access Point and other Health Care Professionals .

The triage nurse will take all new referrals for the local Community Nursing Teams in Cannock Chase, from all health care professionals, surrounding hospitals and G.P. practices and obtain all relevant information and documentation relevant to the referral.

The post holder will require excellent communication and listening skills to be able to manage all referrals, and forward them to the correct team to ensure a seamless process for all of the referrals. IT skills would be valuable but In house training will be given on MPFT systems.
