Southern Health is one of the largest Foundation Trusts in the UK, specialising in mental health, adult and child community health and learning disabilities.


We are seeking an enthusiastic registered Nurse to join our Urgent Community Response (UCR) team delivering care to patients closer to home and preventing unnecessary hospital admission across North Hampshire. The Nurses role is an innovative and dynamic way to work with people to support them to receive high quality care in their own home, and reduce avoidable admissions.

The Frailty nurse will work closely with the Advanced Clinical Practitioners (ACPs), Consultant Practitioner and GPs to provide fast reactive services for patients with exacerbation of long term conditions and/or frailty, ensure rapid delivery of treatment, care planning and admission avoidance where appropriate.

They will provide assessment and care planning including history taking, physical assessment and treatment planning, without direct supervision, and delegate appropriate tasks to Associate Practitioners and Health Care Support Workers and offer clinical supervision to support them in their role.

Our UCR team is developing 4 defined pathways delivered over 7 day working, 8am-8pm. These pathways consist of:

  • Rapid response admission avoidance
  • Falls Car admission avoidance
  • Front door Frailty Team (ED)
  • NHS 111 admission avoidance

Rapid response focuses on preventing avoidable hospital admissions for patients with acute ill health conditions by providing holistic assessment with 2 hours of referral and treatments over a 2 week pathway, typically referred from primary care.

The falls car consists of an Advanced Frailty Therapist or Nurse alongside a Paramedic and responds to referrals directly from 999 for patients who are triaged as low risk of conveyance but require physical assessment and support with functional needs that would, if not addressed, put the patient at risk of hospital attendance through loss of independence and/or falls.

The Front door frailty team (HHFT) is based in Hampshire hospitals and aims to support same day turn around to prevent avoidable admission.

111 admission avoidance pathways are currently in the development phase

The UCR team works in an integrated way with the Front Door Frailty team within ED at Hampshire Hospitals NHS Foundation Trust and with South Central Ambulance Service.

The role allows you to work autonomously and implement a breadth of interventions, while having the support of a wider multidisciplinary team, including nurses, AHP’s and Consultant practitioners.
