Part Time 22.5hrs

 Substantive Post SE Locality Integrated Care Team

 Southern Health Foundation Trust has exciting opportunity for an experienced Nurse to join the Havant Community Care Team. Working in the community you will be providing care to patients’ with complex care needs, enabling patients to receive the care they need in their home.

You will be joining a leading organisation that is both forward thinking and innovative. Southern Health Foundation Trust is committed to improving the quality of community health services across Hampshire and takes pride in rewarding the success of its staff.

We are looking for enthusiastic, caring & motivated professional with excellent clinical, managerial & communication skills, who has the ability to work autonomously, supporting the team lead to manage the case load and staff development.

The post holders must have relevant experience in health care and demonstrate a proactive and positive attitude to learning the skills required to fulfill the role.

Access to a vehicle and possession of a full driving license is essential.
