MLCSU Continuing Healthcare Team (Staffordshire & Stoke on Trent) are committed to ensuring excellent healthcare for all individuals. Our highly skilled and passionate workforce achieve this by working closely with independent care providers, social care and NHS colleagues. Individual commissioning and personalisation are at the heart of our service.

We require a number of motivated and experienced nurses who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC) including the promotion and review of Personal Health Budgets.

The successful candidates will need to be resilient, have attention to detail and a positive approach. You will have knowledge and understanding of the National Framework for NHS Continuing Healthcare (Oct 2018) and NHS Funded Nursing Care, excellent communication and problem-solving skills, have a proven ability to produce high quality documentation whilst working to deadlines and targets. Current NMC adult registration is essential for the role as is the ability to travel both locally and occasionally nationally.
