An exciting opportunity has arisen for an experienced and highly motivated Registered RGN or dual registered RMN/RN Nurse to work as a CHC Nurse Assessor/Case Manager in the South Warwickshire area.

A key role within the development of wider integrated healthcare works streams, this role supports the clinical commissioning of care and services for people with a diverse range of Mental Health needs.

Embedded within the operational structure of the new Coventry and Warwickshire CCG, Arden and GEM CSU provide both clinical and administrative support to the Continuing Healthcare Team for South Warwickshire. This is a permanent, community-based post where the emphasis and values are the team are motivated by quality and patient experience.

For the right candidate, this role offers the opportunity to develop and enhance clinical assessment, critical thinking and decision-making skills. Integral to the post, you will also develop complex case management experience, communication and leadership skills within a highly motivated and supportive team.

You will be working autonomously alongside other specialist Continuing Healthcare Nurse Practitioners in Mental Health, Learning Difficulties and Autism, and Occupational Therapies. Your role will also include close working relationships with the wider community and primary healthcare team providers and Social Services. This role offers the right candidate excellent peer support and opportunities for ongoing professional development.

With a population of 280,000- and 33-member GP practices, located within the districts of Warwick, Leamington Spa and Stratford-on-Avon, the South Warwickshire area is a very attractive place to work. The CCG has stable and committed clinical and managerial leadership and is focused on improving health outcomes for the local population.

As a Band 6 CHC Nurse Assessor/Case Manager you will be responsible for recommending the eligibility for NHS Continuing Healthcare funding for local residents. This will be through completing a range of robust clinical assessments, following the NHS Continuing Health Care Framework. Where people are not eligible for NHS Continuing Healthcare you will also be recommending how any health needs can be met by existing statutory services or, if necessary, recommending joint funding arrangements with the Local Authority.

You will be responsible for managing your own small case load and ensuring your patients and their families/advocates are appropriately supported and engaged throughout the CHC process to enable the optimum, timely provision of appropriate, high quality care. You will develop your own strong links with health and social care colleagues to ensure effective multi-disciplinary working. You will also develop a strong understanding of local NHS and independent sector service provision in individual’s own homes and in care homes.

Previous experience of CHC is not essential for this role, as full training and support will be given as part of a comprehensive induction. Every team member also benefits from regular 1:1 support and protected time for clinical supervision, as part of their own personal development plan.

 

Reporting to a Band 7 Clinical Commissioning Practitioner, the Band 6 CHC Nurse Assessor will be:

  • Highly motivated and keen to learn/develop knowledge and skills - personally and professionally.
  • Familiar and comfortable with quality assurance processes and committed to Continuous Quality Improvement, locally and strategically.
  • Accustomed to working independently, with a high degree of professional autonomy.
  • Motivated and able to manage own time and workload effectively.
  • Reliable and comfortable working at pace, to meet key targets and deadlines.
  • Have excellent listening, verbal and written communication skills with attention to detail and an ability to problem solve in a calm and effective way.
  • Be confident with face to face and virtual working, including completing assessments in patient’s own homes, nursing/residential care homes and supported living accommodation.
  • Be confident and competent using IT - including Microsoft programmes, basic computer literacy and typing skills.
  • Be comfortable and motivated to work with people who will have a wide variety of ages and individuals with a wide range of complexity in their clinical conditions, symptoms and needs, cultural and personal expectations and values.
  • Have a strong value set around transparent, inclusive, holistic assessment and actively promotes excellence for the patient/carer experience in all interactions.

Access to your own transport and an ability to drive is essential for this role.

A Laptop and IT equipment are provided as part of the role.
