Community Mental Health Services in North Lincolnshire have recently benefitted from over £1m investment. This will see specialist mental health services transform over the next three years with improved connections with other healthcare agencies in the area. This development forms part of our wider plan to transform mental health services which has already seen the introduction of a new primary care mental health service, has established a 24/7 acute liaison services, and improved crisis services.

The additional investment has allowed us to introduce this new role into our Community Mental Health Team. As the Clinical Nurse Specialist for the Older Adult Community Mental Health Team and the Memory Team.

You will be responsible for:

•    Providing clinical leadership and practical support and advice to the team members.

•    Contributing clinical expertise to the team with regular clinical meetings and support the continual development of the service.

•    Planning, organising, undertaking, and documenting full bio-psychosocial assessment for patients referred to service, then designing, delivering, and evaluating a plan of care to meet the identified needs.

•    Initiating, monitoring, and reviewing prescribed medications under the supervision of the clinical supervisor in line with Trust policy.

Applicants should be a registered Mental Health Nurse, with evidence of post registration training preferably in older people’s mental health. Experience of coaching and mentoring is essential as this role will be responsible for supporting and developing the skills and knowledge of colleagues with the team.
