TRANSITIONS TEAM – DONCASTER

Band 3 Support Time Recovery Worker

37.5 hours – Fixed Term until 31/3/22

As part of the Community Mental Health Transformation Programme, Doncaster Care Group is setting up an innovative new Transitions Team. This occupational therapy led service will provide effective and supportive interventions to aid people in moving from secondary services back to primary care, ensuring they have what they need to manage their mental health successfully and feel connected to their communities. It will also offer input to those in primary care who are not in crisis, but due to their mental health needs are having issues with their functioning and briefly need therapeutic support to prevent escalation to secondary services.

This post offers an excellent opportunity for a Support Time Recovery Worker wishing to join occupational therapists in supporting people with mental health needs to improve their community integration and collaboratively set up sustainable, forward thinking management plans. The post holder will also contribute to the construction and development of the new Transitions service in conjunction with professional colleagues, experts by experience and carers. The post holder will be overseen by a Band 7 Clinical Specialist OT and Band 6 Specialist OT, taking on specific pieces of delegated work within therapeutic assessments and interventions.

Based in the community in Doncaster, the post holder will be working with the Community Mental Health Teams, GP surgeries, third sector and community organisations to support the smooth transitions of people aligned to their levels of need.

  • You will have experience of accessing or assisting access to health services, preferably in mental health settings
  • You will need good awareness of the needs of people with mental health issues
  • You should be a team player, and a confident and effective communicator with service users, carers and staff of all professional groups as well as with external partner agencies.

Support will be offered through regular clinical and managerial supervision, in-house and external training and an individual approach to your future developmental needs.

Due to the short-term nature of this contract in the first instance, secondment arrangements can be considered. If you are interested in this, please seek provisional agreement from your line manager before applying.
