Our Team is based out of Priorslegh Medical Centre in Poynton, and cover Bollington, Disley and Poynton.

Successful Applicants;

*will have a level 6 or level 7mentorship qualification and recent experience mentoring a pre-registration student nurse.

* Have completed their V100/V150 and would like to complete the V300 prescribing.

* Have sound knowledge of Tissue Viability policy and experience of treating patients on the Leg Ulcer Pathway.

* Experience of Palliative and Bereavement Care, including NVOED (or be willing to attend this course).

* Funded training opportunities tailored to your individual learning needs.

* Varied exposure to a range of clients with differing needs ranging from wound dressings to end of life care and everything in between.

* Our dynamic management team are supportive and open to ideas and innovation.

This post will provide the opportunity to take forward the expanding role of community nursing within the multi-disciplinary neighbourhood teams to meet the long term condition agenda and the needs of our elderly population. It is expected that you will have an understanding and commitment to the modernisation of community nursing.

You will need a sound understanding of the current primary care agenda, including the management of long term conditions and role as care coordinator, with excellent communication skills and effective leadership and decision making skills. You will need to be an innovative individual able to work autonomously whilst promoting a positive and supportive learning environment for team members and students.

You must possess effective communication and negotiation skills. As advocates for our patients we need you to have the ability to inspire people at all levels.

Travel is an essential part of this role; please demonstrate on your application form how you would meet this requirement.

This post is Fixed Term or a Secondment for 10 months

Hours per week: 37.5
