In LPFT we are forging ahead in our community transformation plans that aim to offer a dynamic, localised approach that is responsive, evaluated & tailored to local need, targeting health inequalities. This role is significant in moving towards an integrated care system approach and successful candidates will be trailblazing this new approach.

The Role:
Whilst based within the locality Integrated Place Based Team in Boston you will be assigned to the Primary Care Network's (PCNs) to work autonomously in GP Practice’s to provide mental health leadership and implement clinical processes. In addition to undertaking mental health assessments you will take on a caseload of highly complex service users to provide therapeutic interventions and coordinate their care. Ideal candidates will also be qualified to prescribe and support with medication changes.

Whilst a full Job and Person specification is attached to this advert, key elements of the role will include (but not be limited to):
•Providing specialist clinical knowledge and interventions to service users in primary care setting. This will include working with the practice team to support service users with highly complex mental health needs/serious mental illness.
•Developing and implementing clinical processes that contributes towards the continuous improvement of mental health provision across the primary care network (multiple GP practices)
•Providing best practice training, mentorship and guidance to mental health practitioners and other roles within the primary care/ neighbourhood MDT
•Managing a caseload of highly complex service users, working closely with the locality secondary mental health service, to maintain care plans and avoid escalation to: It is expected that this will equate to 50% of the working week.
•To perform the duties of a non-medical prescriber, ensuring that appropriate medication is prescribed, monitored and administered in line with current best practice and the Trust Medicines Management Policy.

The Successful Candidate will:
•Have the qualifications and experience as highlighted in the Person Specification.
•Be an experienced clinician with proven skills in working with people who have a range of highly complex mental health needs, demonstrating practice that is evidence-based and innovative.
•Preferably have some experience of working closely with or within primary care.
•Demonstrate adaptability, excellent communication skills and self-motivation.
•Be able to demonstrate appreciation of service users needs and a track record of improving care

The post holder will be encouraged to contribute to service development, and there will be opportunities for CPD and clinical supervision as professional development is actively encouraged.

If successfully shortlisted you will be invited to a two stage process (on 2 separate dates). The first stage will be a presentation to a multi-disciplinary panel and the second stage will be a formal interview. Dates will be confirmed to candidates once shortlisting has taken place.

We offer 24 hour services within our Inpatient, Crisis and SPA teams, if you are successfully appointed to posts in these areas or others that might have specified, you will be required to work unsociable hours and shift patterns.
