An exciting opportunity has arisen for an experienced Mental Health Nurses to apply for Band 6 CPN post in the Grantham & Sleaford Mental Health Team based at The Sycamore centre in Grantham. This is a permanent full time position working with service users in the Grantham and Sleaford area. The CMHT ‘s are part of the secondary care services, providing assessments and treatment packages for a range of service users aged from 18 years, with complex mental health needs. The teams use care programme Approach (CPA) and have a focus on recovery and social inclusion.

The successful post holder will maintain a clinical case load, fulfilling all the roles and responsibilities of a Care Coordinator. They will work effectively in the multidisciplinary team to provide high quality mental health assessments, treatment plans and manage risk. This work will involve patients and their carers in the delivery of evidence based interventions/treatments, recovery focused work. You will need to demonstrate enthusiasm and commitment to high quality care as well as possessing good therapeutic and interpersonal skills. The post holder will be required to work autonomously as well as within the MDT and with other LPFT teams , private healthcare providers and outside / voluntary agencies.

The role requires that you will visit people in their own homes and communities as well as providing appointments and initial assessments at team base and on occasion other team bases.

You will have a strong sense of initiative, excellent team working ability and be an excellent communicator. You will be able to demonstrate knowledge of nursing interventions and community assessment skills. A willingness to undertake training relevant to the post is expected.

The successful applicant must be a qualified Mental Health Nurse registered with the Nursing & Midwifery Council/Professional Health Council. A full UK driving license is an essential requirement as is access to the use of a car for work purposes.

We offer 24 hour services within our Inpatient, Crisis and SPA teams, if you are successfully appointed to posts in these areas or others that might have specified, you will be required to work unsociable hours and shift patterns.
