We are looking to recruit 3 WTE Social Prescribing Linked Workers

Social prescribing Linked Workers give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support.

Social Prescribing Link workers also support existing community groups to be accessible and sustainable, and help people to start new groups, working collaboratively with all local partners.

Social prescribing works for a wide range of people, including people:

  • with one or more long-term conditions
  • who need support with their mental health
  • who are lonely or isolated
  • who have complex social needs which affect their wellbeing

The person

You must be a good listener, have time for people and be committed to supporting local communities to care for each other. You should have experience of working positively with people facing complex social and emotional challenges. You will have great interpersonal skills in supporting people, community groups and local organisations. The post holder will work with a diverse range of people from different cultural and social back grounds. The ability to work confidently and effectively in a diverse and sometimes challenging environment is essential. 

Service Hours 

08.00-20.00 Monday-Fridays
