Care Coordinator Band 6 full time permanent post.

An excellent opportunity has arisen for an enthusiastic female nurse to join a dynamic and forward thinking Mental Health Team.

The Assertive Outreach Team provides intensive support to individuals who are experiencing severe and enduring mental health problems, often leading to multiple and complex needs. Care and support is offered in line with the CPA approach and delivered in service-user’s homes or other community settings with a recovery focus at its core.

The role involves been responsible for a caseload of patients with complex needs working directly with the patient, their families and supporting services to provide an holistic approach. While you will be a named care coordinator for your caseload we have a strong team approach ethos and applicants must show strong team working skills to meet the demands of our service. Applicants must have a flexible and innovative approach to promote engagement with services and the recovery process.

You will be a qualified mental health nurse with experience of working within mental health services. You will have a robust understanding of severe and enduring mental health conditions and will be able to demonstrate Knowledge and practice of Psychosocial Interventions. You will have experience of working within the CPA framework and will have knowledge of your responsibilities as a professional within the safeguarding agenda. You must have excellent risk assessment and management skills. Applicants will need to have a strong desire to work in a challenging community setting with the aim of improving our patient’s quality of life.
