Band 7 District Nurse / Team Leader

We are currently recruiting for a dedicated and motivated Band 7 District Nurse and Team Leader to join our community nursing team in the Epsom Primary Care Network. The team is based at New Epsom & Ewell Community Hospital, Epsom. The Community Team visit patients in their own home as well as care homes. The community nursing service works over a 7 day week (9am to 6pm) and includes weekend and bank holiday working.

The District Nurse & Team Leader will provide clinical leadership and case management within the Community Integrated Team and be responsible for the assessment of needs of the patient and the development, implementation and evaluation of a program of care which is person centered. The successful candidate will supervise a team of nurses and HCAs, facilitating training and development of this staff in addition to delivering and maintaining a high standard of clinical and professional Nursing Care.
