We provide a community specialist service within Nottingham University Hospital’s Emergency Department and the Assessment Areas as well as working as an integral part of the Frailty Pathway.

The service is aimed at supporting the Comprehensive Geriatric Assessment (CGA) as early as possible. This involves starting within the Emergency Department and working alongside a specialist multidisciplinary team to determine the best management, treatment and discharge plan for each patient. This will be working closely with Occupational therapists, Physiotherapists and Nurses within the service, as well as Consultants, Doctors and Advanced Practitioners and a range of Health and Social Care Professionals.

We are seeking enthusiastic and dedicated Assistant Practitioners with a sound knowledge and experience of working with complex, frail and older people that are able to manage a caseload that consists of a wide range of cases for people presenting with a plethora of multi pathologies, as well as challenging medical and social circumstances.

Providing holistic assessments and a range of skills within a competency framework, the Assistant Practitioner will be expected to complete initial assessments and follow ups under the guidance and supervision of the Clinical staff.

You will provide community knowledge and skills whilst assessing and recommending appropriate discharge support.

You will need to be dynamic, flexible and have a wide range of problem solving skills.

You will need excellent assessment and intervention skills in order to plan, develop and evaluate goal focussed rehabilitation.

You will need excellent communication and interpersonal skills. You should be able to prioritise your workload, manage your time effectively, and work flexibly within the team.

Upon successful appointment you will complete an induction period and our holistic worker competencies in order to support your development.

You will receive ongoing supervision from a clinical member of staff directed and guided by your needs and be expected to participate in staff development programmes.

You will work as part of an internal rota system between 7.30am – 8pm Monday – Friday

And 8am-6pm on weekends and bank holidays.
