Adult Mental Health Services are looking to recruit a full time 37.5hrs per week Patient Discharge Assistant.

An exciting development opportunity has arisen for an enthusiastic motivated individual within the North Lincs Care Group. We are looking for a discharge assistant to work with our Discharge co-ordinator to support discharges from our inpatient wards at Great Oaks Hospital.

North Lincs inpatient services are part of RDaSH Foundation Trust, which is committed to providing exceptional quality and efficient services and supporting staff to deliver these services to the highest standards. It is a diverse business unit which includes community and inpatient services.

The position is for a discharge assistant to work across the inpatient bed base at Great Oaks. The successful candidate will support the multi-disciplinary team to manage the discharges within service, and ensure that they are safe and effective.

We are looking for a candidate who is motivated and enthusiastic with advanced communication skills and an ability to demonstrate effective leadership qualities in order to deliver high standards of care. The candidate must have good problem solving abilities, to be able to work as part of the team as well being autonomous with your practice.
