Within our Adults Southampton Community Integrated Services team, we have an exciting opportunity for a talented and experienced Nurse who has the desire to move into a senior clinical leadership role as a Consultant Community Nurse

Working alongside the our Service Manager and Clinical Lead (Consultant Geriatrician), this is key leadership role that will need to lead from the front to enable improved health outcomes and promote health and wellbeing for our patients and their families. It is essential that as a leader in this team that you can easily demonstrate care, compassion, competence, communication, courage and commitment through your development of close working relationships with all Solent’s existing services and multi-agency partners in social care, secondary care, primary care, GP federations and the voluntary sector (particularly the local Hospice).

Understandably you need to be highly competent, visible, and experienced practitioner who is driven, ambitious and passionate about patient care with a desire to influence at the highest level both clinicians, operational leaders and our strategic partners.

For anyone interested in this role, it is essential that you have an extensive portfolio of clinical leadership and can demonstrate capability across the four key domains of advanced practice. Below is the evidence of the demonstration required for suitable applicants

Clinical Practice - The post holder will provide advanced and expert level comprehensive assessments, clinical consultancy, case management treatment and education. This will predominantly be delivered within the community setting, though the scope of practice may extend into inpatient and clinic settings to support with complex cases and patient pathways.

Leadership and Management - undertake professional, governance and service leadership, consultancy and strategic whole system work alongside partnership agencies, with close working relationships to Primary Care Networks including referrers, commissioners and provider organisations.

Provide clinical/professional supervision and support for community nursing and advanced practitioners within the community not purely within their own profession.

Education and Training - Lead on the development of educational forums, developing and delivering competency frameworks within the field of Aging Well, Frailty and Community Nursing.

Research, Evaluation and Service Development - To lead on growing a culture of embedding evidence-based practise, clinical research, supporting a research portfolio for studies within the speciality.

They will have undertaken or be willing to undertake Qi and research to ensure we are developing and delivery the most up to date levels of care to the local population.

Overall, the successful candidate along with the Service Manager and Medical lead will provide positive inspirational leadership, connecting patients, staff and partners with the vision and values of Solent NHS Trust and Adults Southampton’s Integrated services.

The Adults Southampton service line provides personalised care across the Southampton system through its four community wards, specialist services (Urgent Response, Neurological Rehabilitation and Long Term Conditions) and the Community Integrated Services within which this post will sit. The Community Integrated Services consist of Community Nursing, Community Therapy, Consultant Geriatricians and the integrated Community Social Care teams. These work from three localities across the city but are increasingly linked with the five city PCN’s.
