Working Pattern: 1 full time permanent, 2 full time fixed term for one year

Please note this post is based in North Lincolnshire.

The community staff nurse will work as part of an integrated team contributing to the family health needs assessment when working with families, children and young people age 0-19 years and up to 25 for young people with SEND. The post holder will work in partnership with the family and other agencies to develop, implement and evaluate packages of care on an individual basis and with groups.
