We are seeking experienced nurses to provide complex care and treatment to both adults and older people living at home who have long-term or complex health needs.

You will work alongside a number of specialist community services including Palliative Care, Proactive Nursing Services, Bladder and Bowel, Respiratory, Diabetes as well maintaining established pivotal relationships with our primary care colleagues.

Role responsibilities

  • First contact: acute assessment, nursing diagnosis and care, treatment, and referral
  • Palliative care: symptom management including assessment of acute symptom onset, care for the patient and their family/carers
  • Continuing care: intermediate care, rehabilitation, chronic disease management
  • Public health: health protection and promotion programmes that improve and reduce inequalities
  • To actively safeguard and promote the welfare of children and adults

Working hours

  • Core - 08:30 – 16:30
  • Mid Shift – 10:00 - 18:00 or 12:00 20:00 (average 4 per month)
  • Twilight – 14:00 - 22:00 (average 4 per month)
  • Weekend working is 1 in 4
