Advanced Clinical Practitioner

gtd healthcare are seeking to improve the way people receive urgent primary care, advice and treatment and are looking to recruit advanced clinical practitioners (ACP) to work within our Digital Hub and Pre Emergency Department (ED) streaming service based in the Oldham locality. The role is deemed a key asset in ensuring patients are seen as quickly and safely as possible, in the right place and at the right time.

Our new and innovative Digital Hub is striving to make a real difference to the health and wellbeing of the patients of Oldham and surrounding communities and you could be a key part in improving the patient journey for the population of Oldham.

You will be provided an opportunity to work within a service that will contribute towards transforming how the local population accesses their healthcare. Furthermore, you will have a key role in the delivery of excellent clinical care and be a key member of the senior clinical team, identifying opportunities to innovate and work differently to meet the demands of the changing healthcare environment.

As an ACP working within the Oldham locality you will have exposure to a multitude of streaming pathways, a broad Directory of Services (DoS) along with :
• Direct access to a wide range of community-based services
• Referral rights to secondary care
• Access to technology to support telephone and video consultations
• Facilities to see patients face to face

In addition, the pre ED streaming arm of the service includes working closely with The Royal Oldham Hospital to identify patients with primary care presentations who can be streamed away from ED

To express your interest, you must meet the following criteria:
• MSc in Advanced Practice or credentialing qualification (or equivalent experience) is required
• A leadership qualification is desirable
• V300 non-medical prescribing qualification (if you are about to embark on NMP training or currently undertaking NMP training we would still like to hear from you)
• Experience of primary care or urgent care
• ED experience is an advantage but not necessary
• A ‘can do’ attitude
