We have an exciting opportunity for you to join an established Specialist palliative Care service in South West Herts based at Hemel One, Hemel Hempstead.

This role is full time

 

You will be:

  • A qualified nurse with appropriate skills and experience working in the palliative / End of life arena
  • A well organised. Flexible professional
  • Compassionate to the needs of palliative and end of life care patients/carers
  • An understanding of the 4 organisations who make up the palliative care providers in this divison
  • Used to working collaboratively as part of a multidisciplinary team
  • Have a wide range of IT skills and experience with use of Systmone
  • Excellent communication skills
  • Knowledge and skills of community nursing
  • Triaging skills to ensure the best outcome for our patients
