This is your opportunity to join the clinical inreach team at Nottingham CityCare Partnership, providing a comprehensive and patient centred approach to admission avoidance and supported transfer of discharge. Surrounded by the best clinical and support staff, you will be creating forward thinking and innovative approaches to managing people with multiple pathology and complex health needs working with our Nottingham University Hospital Trust partners.

We are looking for an experienced and skilled Physiotherapist to work in our fast paced and well-established Integrated Discharge Team. We provide a specialist service within Nottingham University Hospitals Emergency Department and assessment areas.

Our remit is to use our specialist assessment and knowledge of community services to work with the MDT and determine appropriate management and treatment for patients. The team ethos is: Ongoing treatment to be provided by the right service, at the right time, in the right place without delay.

You will need excellent assessment and intervention skills in order to plan, develop and evaluate goal focussed rehabilitation, as well as complex discharge planning experience.

You will need to demonstrate high standards of evidence based clinical practice. You must have excellent interpersonal and communication skills. You should be able to prioritise your workload, manage your time effectively, and work flexibly within the team.

You will need excellent supervisory skills, up to date knowledge of staff appraisal and performance management experience.

You will receive supervision from a senior member of staff and be expected to participate in staff development programmes.

Nottingham CityCare Partnership and Nottingham University Hospitals’ Services operate a 7 day service 7.30am – 8pm including weekends and bank holidays as part of an internal rota.
