An opportunity has arisen for a highly skilled, enthusiastic, conscientious, experienced and motivated occupational therapist or physiotherapist, with a keen interest in the holistic working model, to join the Urgent Care and Reablement service operating across Nottingham City.

The service consists of multi-disciplinary team that includes Physiotherapists, Occupational Therapists, Nurses, Registered Medical Technician, Assistant Practitioners and Rehabilitation Support Workers. The aim of the MDT is to promote independence through rehabilitation and crisis management with citizens in their own home or within a short term care setting to prevent unnecessary hospital admissions and support timely hospital discharges.

We are a dynamic, developing organisation with well recognised rehabilitation services providing education, advice, support and rehabilitation for patients across health, social care, care homes and the independent sector.

An understanding of Health and Social care provision and ability to work closely with partner organisations and other stakeholders is essential. You will need excellent assessment and intervention skills in order to plan, develop and evaluate goal focussed rehabilitation and Reablement programmes, as well as complex discharge planning experience.

You will need to demonstrate high standards of evidence based clinical practice. You must have excellent interpersonal and communication skills. You should be able to prioritise your workload, manage your time effectively, and work flexibly within the team.

Within the team we also manage the community triage hub which facilitates all hospital discharges to our community rehab and assessment beds, and coordinates home discharges for patients who need short term support from the team.

You will use your clinical skills to triage referrals, liaise with wards, community bed staff and hospital discharge teams to facilitate timely discharges from the acute setting, and support the flow of patients through the system. Excellent interpersonal, organisational and communication skills are required, and you should be able to respond pragmatically to rapidly changing demands and circumstances.

The role includes delivery of efficient and effective services in line with local, national and professional agendas.
