We have the following permanent opportunity as Band 6 Nurse to work within the Anticoagulation Team.

Are you someone who is motivated, assertive with attention to detail with an interest in anticoagulation?

The Anticoagulation Nurse role entails specialist aspects of patient management which include health assessment and management of patients who require anticoagulation therapy. Our aim is to provide a seamless service and expert care for patients requiring anticoagulation in the community setting.

You will manage patients that require anticoagulation medication both short term and long term reviewing treatment plans as appropriate. This role involves empowering patients to manage the plans effectively and regular liaison and communication with wards and departments with the hospital, GP's and Community health teams in Primary care is required

If you have the following, we would like to hear from you.

  1. Experience of working in an anticoagulation clinic
  2. 3-5 years of post-registration experience working in an acute nursing environment
  3. Ability to work autonomously and to exercise initiative.

We welcome applicants preferably with experience in this field +/- use of 4sDAWN dosing system but this is not essential as you will be well supported by the team with a detailed learning program to suit your needs
